29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition where it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. a) Malm Electrical Contractors, LLC Located at 200 Peridot Ave., Loveland, CO 80537: On and preceding July 6th, 2018, the employer did not ensure that employees were provided with, and utilized, adequate personal protective equipment while performing electrical work. Employees removed electrical box covers, and tested 277 volt lighting circuits, as well as other 480 volt circuits, without adequate personal protective equipment. This condition exposed employees to electrical shock hazards.
