29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about July 6, 2018 at the address of 6363 Monroe Street, Sylvania, OH:  The employer did not ensure that employees were provided with safe access to the upper landing surface of a one story structure by extending the ladder side rails at least three feet above the surface to be accessed.  Employees were exposed to a 12 ft. fall hazard.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) On or about July 6, 2018 at the address of 6363 Monroe Street, Sylvania, OH:  The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.   Donald Siedlecki was previously cited for a violation of this Occupational Safety and Health standard 1926.20(b)(1) which was contained in OSHA inspection number 1169219, citation number 1, item number 1a and was affirmed as a final order on September 26, 2016, with respect to a workplace located at 8907 Red Hawk Ct. Sylvania, OH.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On July 6, 2018, at the address 6363 Monroe Street Sylvania, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces.   Donald Siedlecki was previously cited for a violation of this Occupational Safety and Health standard 1926.501(a)(1) which was contained in OSHA inspection number 1169219, citation number 1, item number 1c and was affirmed as a final order on September 26, 2016, with respect to a workplace located at 8907 Red Hawk Ct. Sylvania, OH.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)  On July 6, 2018, at the address of 6363 Monroe Street Sylvania, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a commercial type structure.  Employees operating in this condition were exposed to a 12 ft. fall to the ground below.  Donald Siedlecki was previously cited for a violation of an equivalent Occupational Safety and Health standard 1926.501(b)(13) which was contained in OSHA inspection number 1169219, citation number 1, item number 1b and was affirmed as a final order on September 26, 2016, with respect to a workplace located at 8907 Red Hawk Ct. Sylvania, OH.
