29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about June 14, 2018, in the area on the North side of the building, where employees working from JLG 400S aerial lift were exposed to a fall hazard while changing out the plywood and weather barriers on the building without a body belt with lanyard connected to an anchorage point on the boom or basket of the lift.
