29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling: a) On or about June 29, 2018, and at times prior, the controlling employer, Colorado Structures, Inc. d/b/a CSI Construction did not ensure subcontracted employees working on a scaffolding system were protected from fall hazards of approximately 14 feet while engaged in masonry operations on an elevator tower. The scaffolding system was approximately 24 inches from the elevator tower wall; no guardrail or personal fall protection systems were used to protect employees fall hazards.
