29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) The employer did not ensure that employees working on the ground directly under employees working approximately 9 feet above them; were wearing protective helmets thereby exposing employees working on the ground to a struck-by hazard from falling materials and/or tools.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On July 5, 2018 two employees were observed framing the roof of a house being built at 809 Evergreen St, Princeton, TX approximately 9 feet or more above the concrete floor below and were not protected from a fall hazard by using either guardrail systems, safety net systems, or personal fall arrest systems.
