29 CFR 1926.352(d): Suitable fire extinguishing equipment was not immediately available in the work area(s) where welding, cutting, or heating was being performed: On or about July 6, 2018, at the job site an employee was exposed to fire hazards while performing welding operation in a lifting platform mounted to a forklift truck and raised to a height approximately 30 feet without a fire extinguisher provided on the lifting platform for immediate use.
