29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Facility, 921 Berlin Plank Road, Somerset, PA:  On or about July 8, 2018 the employer had not developed a procedure to control the energy for the tag line axle when employees are engaged in vehicle maintenance.  An employee did not release the air pressure and lower the tag line axle to the ground prior to beginning a repair.  The airline developed a hole and the pressure was released which resulted in the axle coming down and trapping the employee.
