The Yale battery powered pallet jack (Model: MPB040ACN24C2748, S/N: A82N21300V) was being operated without having completed the required training.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: On or about 07/09/2018, Fabritec Industries Inc., located at 1510 Fairview Ave., St. Louis, MO, had various electrical breaker boxes blocked for safe and easy accessibility. One such electrical breaker box was located along the south wall in the Sewing Department. Other electrical breaker boxes were located along the North wall in the Sewing Department. These electrical boxes were blocked by multiple boxes.
