Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. (a) Quickdrain USA at 2597 W. 64th Ave., Denver, CO 80221: On or before 6/14/2018, the employer did not report an amputation that occurred at work within 24 hours. The employer reported this incident to OSHA after five calendar days.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Quickdrain USA at 2597 W. 64th Ave, Denver, CO 80221: On and before June 14, 2016 the employer did not ensure the points of operation were properly guarded to prevent employees from having any part of their body in the danger zone during operation. This condition exposed the employees to amputation hazards. This condition includes, but is not limited to, the following equipment: (1) Router table Inadequately guarded point of operation presented amputation hazards. The opening under the blade guard measured 1-1/8 inches high. (2) Table saw Unguarded point of operation presented amputation hazards. (3) Radial arm saw Unguarded point of operation presented amputation hazards.

29 CFR 1910.134(c)(2)(i): While the employer did provide respirators at the request of employees and permited employees to use their own respirators, the employer did not determine that such respirator use will not in itself create a hazard. If the employer did not determine that any voluntary respirator use is permissible, the employer did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"); (a) Quickdrain USA at 2597 W. 64th Ave., Denver, CO 80221: On and before June 14, 2018, the employer did not ensure that the employees voluntarily using a half face, tight fitting respirator in the production area and while using the router table, table saw, and radial arm saw to cut foam core (ArmaFORM PET GR) were medically able to use that respirator and that the respirator was cleaned, stored and maintained so its use did not present a health hazard to the employee. The employer did not provide a copy of Appendix D to 29 CFR 1910.134 to employees voluntarily wearing respirators.
