29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Affordable Roofing & Restoration, Inc. at 5541 Wetlands Drive, Fredrick CO 80504: On July 9, 2018 the exposing employer did not ensure that employees were protected from injury, in that one employee working on the roof of a two-level residential home was not utilizing a fall protection system. This condition exposed the employee to fall hazards ranging from 20 to 10 feet. (b) Affordable Roofing & Restoration, Inc. at 5541 Wetlands Drive, Fredrick CO 80504: On July 9, 2018 the exposing employer did not ensure that employees were protected from injury, in that one employee working on the garage of a two-level residential home was not utilizing a fall protection system. This condition exposed the employee to fall hazard of approximately 10 feet. (c) Affordable Roofing & Restoration, Inc. at 5541 Wetlands Drive, Fredrick CO 80504: On July 9, 2018 the exposing employer did not ensure that employees were protected from injury, in that two employees tied their personal fall arrest system to the same single-person anchor. This condition exposed the employee to fall hazards ranging from 20 to 10 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Affordable Roofing & Restoration, Inc. at 5541 Wetlands Drive, Fredrick CO 80504: On July 9, 2018 the exposing employer did not ensure that each employee engaged in roofing operations was trained to recognize the hazards of working at elevation and trained in the use of fall protection, in that one employee working on the roof of a two-level residential home was not utilizing a fall protection system. This condition exposed the employee to fall hazards ranging from 20 to 10 feet.
