29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  On or about July 3, 2018, employees were exposed to a fall of approximately 25 feet while installing masonry materials on a new residential apartment building located at 100 Lake Forrest, Hudson, Ohio 44236. The employees were working on all levels of the scaffold and the employer did not ensure that it was inspected by a competent person.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  On or about July 3, 2018, employees were exposed to a fall of approximately 25 feet while installing masonry materials on a new residential apartment building located at 100 Lake Forrest, Hudson, Ohio 44236. The employees were working on all levels of the scaffold and the employer did not ensure that the working levels were fully planked.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  On or about July 3, 2018, employees were exposed to a fall of approximately 25 feet while installing masonry materials on a new residential apartment building located at 100 Lake Forrest, Hudson, Ohio 44236. The employees were working on all levels of the scaffold and the employer did not ensure that the scaffold was tied into the structure to prevent tipping.

The employees were working on all levels of the scaffold and the employer did not ensure that the scaffold was constructed on an adequate foundation.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about July 3, 2018, employees were exposed to a fall of approximately 25 feet while installing masonry materials on a new residential apartment building located at 100 Lake Forrest, Hudson, Ohio 44236. The employees were working on all levels of the scaffold and the employer did not ensure that the scaffold had safe means for employees to access to the upper two levels.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. . On or about July 3, 2018, employees were exposed to a fall of approximately 13 feet while installing masonry materials on a new residential apartment building located at 100 Lake Forrest, Hudson, Ohio 44236. The employees were using an extension ladder to access the 2nd level and the employer did not ensure the ladder extended at least 3 feet above the landing area.
