29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:Note: If the employer could demonstrate that it was infeasible or created a greater hazard to use these systems, a fall protection plan meeting the requirements of paragraph (k) of 1926.502 should have been developed and implemented. At the time of the fall on July 2, 2018, the employee on the north end of the leading edge was not protected from falling by a personal fall arrest system. The employee fell from the leading edge to the floor 47 feet below. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system; At the time of the inspection, July 15, 2018, employees on the low slope Cast Roof were not wearing fall protection at all times. Employees were exposed to unprotected sides and no guard rails, warning line system or safety net system was installed.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: At the time of the inspection, July 15, 2018, it was determined that employees had not been using a personal fall protection system when working on a low slope roof without guard rails, warning line system or safety net system installed. Employees were using a 15 foot rule to determine when fall protection system was required which was not part of the employer's fall protection program. The employee training did not adequately address how employees were to handle using multiple horizontal lifelines and retractable lanyards when working on the low slope roof.
