29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.    At the north east corner of the maintenance shop, employees used a double sided bench grinder and one side was not equipped with a work rest and the other side the work rest was not adjusted exposing employees to caught-in and struck-by hazards.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel.     At the north east corner of the maintenance shop, employees used a double sided bench grinder that did not have tongue guards installed to protect against flying debris exposing employees to struck-by hazards.
