29 CFR 1926.353(b)(1): Except as provided in paragraph (b)(2) of this section, and paragraph (c)(2) of this section, either general mechanical or local exhaust ventilation meeting the requirements of paragraph (a) of this section shall be provided whenever welding, cutting, or heating is performed in a confined space.    On or about July 10, 2018, ventilation was not provided to a confine space area where an employee was welding and cutting.  This condition exposed the employee to health hazards such as but not limited to breathing welding fumes and such as oxygen deficiency.

29 CFR 1926.1203(e)(2)(iii): Before an employee enters the space, the internal atmosphere was not tested with a calibrated, direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order:    On or about July 10, 2018, the employer did not perform a confine space atmosphere test for potential contaminants such as but not limited to welding fumes. This condition exposed employees to atmospheric hazards such as such as oxygen deficiency.

29 CFR 1926.1207(a): The employer did not provide training to each employee whose work is regulated by this standard, at no cost to the employee, and did not ensure that the employee possessed the understanding, knowledge, and skills necessary for the safe performance of duties assigned under this standard:    On or about July 10, 2018 the employer did not provide training in confine space entry with permit required to an employee who performed welding activities inside a manhole.  This condition exposed employee to atmospheric hazards such as oxygen deficiency.

This condition exposed employee to atmospheric hazards such as oxygen deficiency.
