29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: On or about July 10, 2018 the employer failed to get approval for the modified wood platform the employer put on the attachments of the Lull. An employee was standing on the wooden platform dumping used wood into an eight foot high dumpster.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Employees were exposed to falls of approximately 10 feet while on the scaffold platform supported by the Lull and the employer did not ensure employees were protected from falling by personal fall arrest systems or guardrail systems.
