29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. Location: site On or about 6/26/18, A JLG model 1044C-54 all-terrain forklift was operated with hydraulic oil leaks severe enough to saturate the hydraulic hoses, battery and visible spots on the ground below the machine.

29 CFR 1904.32(a)(1): The employer did not review the OSHA 300 log to verify that entries were complete and accurate, and correct any deficiencies identified. (a) The employer did not ensure that the 2017 OSHA Summery log accurately reflected the injury and illnesses on 300 log. The summery only included the days away total and the number of cases for days away (which did not coincide with the 2017 300 log) and did not include the total of other recordable cases. (b) The employer did not ensure that the 2016 OSHA Summery log accurately reflected the injury and illnesses on 300 log. The summery only included the days away total and the number of cases for days away (neither of which accurately reflect the data on the 2016 300 log) and did not include the total of other recordable cases. (c) The employer did not ensure that the 2015 OSHA Summery log accurately reflected the injury and illnesses on 300 log. The summery only included the days away total and the number of cases for days away (neither of which accurately reflect the data on the 2015 300 log) and did not include the total of other recordable cases.
