29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. (a) On July 5, 2018 employees were exposed to a fall hazard approximately 25 feet to the lower ground level by not being protected from falls when working on a pump jack scaffold.

29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories shall be fabricated from metal plates and angles. Each pump jack bracket shall have two positive gripping mechanisms to prevent any failure or slippage. (a) On July 5, 2018 employees were exposed to fall hazard of approximately 25 while working on the pump jack scaffold without brackets or braces fabricated from metal.

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled. (a) On July 5, 2018 employees were exposed to a fall approximately 25 feet by not having the pole legs secured to the structure building at the bottom of the pump jack scaffold.
