(a) Boulder Community Health dba Community Medical Emergency Department, 1000 W. South Boulder Rd., Louisville, CO 80026: On and before July 11, 2018, the employer did not review and update their Exposure Control Plan on an annual basis. The employer provided an plan that was last updated in March 2017.
