29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) On or about July 10, 2018, in the paint and mechanical/fabrication area, the employer failed to ensure that an effective strain release was attached to the plug of the flexible electrical cord of the "Central Machinery" belt and 9" sander model T6852. b) On or about July 10, 2018, in the paint and mechanical/fabrication area, the employer failed to ensure that an effective strain release was attached to the flexible electrical cord at the point of entry into the metal body of the "Central Machinery" belt and 9" sander model T6852.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a) On or about July 10, 2018, in the paint and mechanical/fabrication area, the employer failed to ensure that the space in front of electrical panels was free of obstructions to permit ready access. Tool carts and racks with tubing and other materials were stored in front of the lights panel and circuit #5 switch.
