29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  On or about July 23, 2018 employees were exposed to hazards from an inadequately guarded chain and sprocket under the line 4 accumulator table where the wire mesh guarding had pulled aware from the frame enclosure leaving a triangular opening measuring 5 inches along the vertical edge and 8.5 inches along the horizontal edge.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about July 11, 2018 employees were exposed to hazards associated with improper use of electrical equipment where the employer used extension cords as a substitute for fixed wiring to facilitate the connection of cord and plug connected equipment to branch circuit receptacles.  a.  Strain relief was not provided to insulated, electrical conductors entering a pull box the "Harland" Sirius labeling machine on bottling line 3 and the conduit through which the conductors ran was taped to the pull box where the strain relief failed.  b.  Strain relief was not provided to insulated, electrical conductors entering a pull box attached to a 180 volt electric motor on or adjacent to the "Harland" Sirius labeling machine on bottling line 3.
