29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Wiers Farm, Inc., located at 4465 State Route 103 South, Willard, OH 44890: On or about July 3, 2018, the employer did not adequately train employees to control hazardous energy of the equipment such as, but not limited to, the de-tailer on the Radish Grader Line located in the Packing House. Employees did not control all hazardous energy sources such as, but not limited to, electrical while employees were conducting activities such as, but not limited to, cleaning the de-tailer. Employees were exposed to laceration and amputation hazards while all hazardous energy sources were not controlled while conducting work.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): A) Wiers Farm, Inc., located at 4465 State Route 103 South, Willard, OH 44890: On or about July 3, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees performing activities such as, but not limited to, cleaning the de-tailer on the Radish Grader Line in this condition were exposed to laceration and amputation hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from laceration and amputation hazards while employees conducted activities such as, but not limited to, cleaning the de-tailer on the Radish Grader Line located in the Packing House.

Employees were exposed to caught-by and laceration hazards.
