29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: On or about July 11, 2018, during the waterproofing of an underground parking garage at a worksite located at 12550 Lake Avenue, Lakewood, Ohio, the employer did not ensure two employees inside an approximately 10 to 11 foot deep portion of a trench with a near 90 degree vertical east side wall were protected from cave-ins by an adequately designed protective system. The soil would have been classified as Type A .
