29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a) Exterior, NW Corner of Building - Employees were observed standing on a  tubular welded frame scaffold work platform and the platform was not solidly planked, on or about 07/10/2018.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Exterior - Employee was performing masonry work operations while on a tubular welded frame scaffold and had not been trained to be aware of hazards such as but not limited to; working from a fully planked platform, base plates were missing from the legs, scaffold was not plumb, safe access to the work platform was not provided, and not provided with fall protection, on or about 07/10/2018. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access:    a) Exterior, NW Corner of Building - Employees were using the cross bracing on a tubular welded frame scaffold to ascend and descend to the scaffold work platform that was 18 feet above the ground, on or about 07/10/2018.
