29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:     a) On or about July 10, 2018; second floor; employee was using a surge/power strip to provide power to equipment such as but not limited to circular saws and compressors.      b) On or about July 10, 2018; second floor; employee were using energized electrical boxes that were not properly mounted as they were designed.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:  a) On or about July 10, 2018; second floor area; employees had equipment such as but not limited to the air compressor plugged into electrical boxes that were missing faceplates exposing them to a shock hazard.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about July 10, 2018; second floor area; employees were working near floor openings such as but not limited to the stair opening that was not guarded exposing them to an estimated 10-foot fall to the lower level.      b) On or about July 10, 2018; roof area; employees were working near unguarded wall openings exposing them to an estimated  27-foot fall to ground level.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded:    a) On or about July 10, 2018; third floor; employees that were using full body harness, while working on the porch area, had tied thier lines around the central beam located on the third floor exposing line to being abraded while it moved around the beam.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record that contains the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer and when the employer relies on training conducted by another employer the certification record needs to  indicate the date the employer determined the prior training was adequate rather than the date of actual training.   a) On or about July 10, 2018; porch area; the employer had not verified the fall protection training to employees that were exposed to fall hazards 6-feet and greater as well as methods to abate or protect themselves while conducting their assigned duties.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet)
