29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On or about July 12, 2018, employees engaged in roofing work activities from a height greater than six (6) feet above ground level, were not protected by the use of fall protection. This exposed employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards. On or about July 12, 2018, the employer did not ensure employees engaged in roofing activities who were working at a height greater than six (6) feet above ground level, were provided fall protection training. This exposed the employees to the fall hazards.
