29 CFR 1926.102(a)(1):   Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  Instance a)  4 Nast Road, Building #48, Livingston, NJ.   The employer had not ensured the employee was given eye protection as the worker was exposed to penetrating eye trauma from nails while using the pneumatic coil nailer.                                       Violation observed on or about 07/03/2018.

An employee was exposed to falls of 18'9" from the roof sheathing on the 4 in 12 sloped roof to the ground below as no methods of fall protection were being used. 29 CFR 1926.502(d)(1):   Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials:   Instance a)  4 Nast Road, Building #48, Livingston, NJ.  An employee was exposed to falls of 18'9" from the roof sheathing on the 4 in 12 sloped roof to the ground below as the employee tied the rope lifeline directly to the D-ring of the full body harness which was not a compatible connection.                                      Violation observed on or about 07/03/2018.

An employee was exposed to falls of over 9' from roof sheathing or joists to the second floor below as worker used the wood framing to climb from the roof to the second floor where a ladder should have been used. An employee was exposed to falls from the 40' portable aluminum extension ladder used to access upper levels given that the ladder's rails were bent, a rung was bent and a rung and ladder feet were missing.
