29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices were not singularly identified; were not the only devices used for controlling energy; were used for other purposes; and did not meet the requirements of subparagraphs (A) through (D): On or about July 10, 2018, the employer failed to ensure locks that were being used for the control of hazardous energy control were singularly identified. Locks were assigned to a piece of machinery instead of to an employee. Machine operators apply the lock when cleaning the machinery. Some machines do not have a place to apply a lock so the e-stop is used instead. The supervisor has the key to the locks.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about July 10, 2018, the employer failed to conduct a periodic review of their written control of hazardous energy program and its components. The last review documented was in January of 2017 and there have been changes that affect the program since then, to include but not limited to new employees being added as authorized under the program.
