29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Employees were exposed to cutting hazard and amputation injury while working on and around the Pat Mooney Inc. Horizontal Bandsaw (Model #Jupiter C 1000 and Serial #W00998). The employer did not ensure the bandsaw blade point of operation was properly guarded. (b) Employees were exposed to cutting hazard and amputation injury while working on and around the Wellsaw Horizontal Bandsaw (Model #1000 and Serial #3374). The employer did not ensure the bandsaw blade point of operation was properly guarded. (c) Employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the Standard Industrial Press Brake(Model #AB500-12 and Serial #400929). The employer did not ensure the point of operation was properly guarded. (d) Employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the Accur Press Press Brake(Model #710010 and Serial #3593), and the employer did not ensure the point of operation was properly guarded. (e) Employees were exposed to crushing/pinch point hazards and amputation injury while working on and around the Donewelll Press Brake (Model #H335-14/12 and Serial #384121945). The employer did not ensure the point of operation was properly guarded.
