29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    At the location of 14275 W. 135th St. in Olathe KS, an employee moving and removing stones onto a scaffold with the use of a forklift (Dynalift Telescoping Boom Forklift #553) and employees on the scaffold were exposed to struck-by hazards. The employees were exposed in that the forklift operator was not wearing a seatbelt and did not complete forklift training since 07/31/13.
