29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section: At the job site, 12203 S. Strang Line Rd. Olathe, Kansas, employees engaged in masonry activities from a tubular welded frame scaffold was exposed to fall hazards of approximately 11 feet 8 inches from the working level to the ground below in that fall protection was not provided at the ends of the scaffold. There was an opening measuring 20 inches at one end/corner of the scaffold. Bonilla Masonry, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard number 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1002994, citation 1, item number 2a and was affirmed as a final order on 4/22/2015, with respect to a workplace located at 1191 SW Wanamaker Topeka, Kansas 66612. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
