29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a) 45 Degree Turn, Line 1, 5530 S.R. 6 West, Suite 1, Tunkhannock, PA - The conveyor rollers leading into the 45 Degree Turn were driven by a chain and sprocket system three (3) foot above the concrete floor that was not enclosed on the inner side of the curve exposing employees to an in-running nip point and caught-in hazards, as observed on or about July 12, 2018. b) 45 Degree Turn, Line 2, 5530 S.R. 6 West, Suite 1, Tunkhannock, PA - The conveyor rollers leading into the 45 Degree Turn were driven by a chain and sprocket system three (3) foot above the concrete floor that was not enclosed on the outer and inner side of the curve exposing employees to in-running nip points and caught-in hazards, as observed on or about July 12, 2018. c) Paint Booths 1 and 2 on Lines 1 and 2, 5530 S.R. 6 West, Suite 1, Tunkhannock, PA - The paint booth conveyor rollers exiting the four (4) paint booths were each driven by chain and sprocket systems three (3) foot above the concrete floor that were not enclosed exposing employees to in-running nip points and caught-in hazards, as observed on or about July 12, 2018. In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Line 1, 5530 S.R. 6 West, Suite 1, Tunkhannock, PA - An employee sustained a work-related injury on June 7, 2018 after coming into contact with the ingoing nip point of a chain and sprocket on the operators side of the pallet exit of the first paint booth that resulted in an amputation of the tip of the left ring finger. The amputation was not reported by the employer to OSHA within twenty-four (24) hours, on or about June 8, 2018. In accordance with 29 CFR 1903.19, abatement certification is not required for this violation as the hazardous condition was corrected during the inspection.
