A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. On or about July 10, 2018, at a job-site located at 7TH ST SW at Harrison Ave SW, Canton, Ohio 44706; the employer did not ensure employees had means of access and egress while working inside a trench that had near vertical side walls and a varying depth of approximately 4.11 to 5.1 feet.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about July 10, 2018, at a job-site located at 7TH ST SW at Harrison Ave SW, Canton, Ohio 44706; the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with near vertical side walls and a depth varying from approximately 4.11 to 5.1 feet. The trench was determined to have type "C" soil.
