29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour:  Site- On or about July 11, 2018 employees were not protected from a fire hazard where four compressed oxygen cylinders were stored directly next to a cylinder containing propane.

Site- On or about July 11, 2018 employees working from the roof were not protected from a fall of 11 feet 6 inches where fall protection was not utilized.

Site- On or about July 11, 2018 employees cutting a hole in the roof approximately 6 feet by 4 feet were not protected from a fall of 11 feet 6 inches where fall protection was not utilized.
