29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, were not guarded: a. On July 12, 2018, in the shop, three pallet cutting machines did not have an adequate point of operation guard, exposing employees to an amputation hazard. Pallet Consultants was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1274113, citation number 1, item number 2 and was affirmed as a final order on December 13, 2017, with respect to a workplace located at 5402 W. 1st Street, Jacksonville, Florida 32254.
