Employees were re-roofing a 6/12 pitch roof of a two story residence. Employees accessed the roof using an aluminum extension ladder. The ladder side rails extended one rung past the roof edge. The employer did not ensure that the ladder side rails extended at least three feet above the roof edge. This condition exposed employees to an approximate 11.9 foot fall hazard.
