29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:  (a)  I & D Roofing LLC, at 116 6th Avenue, Superior, CO: On 7/11/18, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  Employees were re-roofing a 6/12 pitch roof of a two story residence.  Employees were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  This condition exposed employees to approximate 16.2 foot and 11.9 foot fall hazards.

Employees were re-roofing a 6/12 pitch roof of a two story residence.  Employees were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  The employees had not been provided fall protection training.  This condition exposed employees to approximate 16.2 foot and 11.9 foot fall hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)  I & D Roofing LLC, at 116 6th Avenue, Superior, CO: On 7/11/18, the employer did not ensure that portable ladders used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  Employees were re-roofing a 6/12 pitch roof of a two story residence.  Employees accessed the roof using an aluminum extension ladder.  The ladder side rails extended one rung past the roof edge.  The employer did not ensure that the ladder side rails extended at least three feet above the roof edge.  This condition exposed employees to an approximate 11.9 foot fall hazard.
