29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms and walking-working surfaces were not kept clean, orderly and in sanitary condition. On or about July 13, 2018, employees were exposed to tripping hazards caused by electrical cords and floor outlets not being properly covered and/or secured underneath massage beds as the employer did not ensure that all walking-working surfaces were kept in a clean, orderly, and sanitary condition. There is no abatement certification or documentation required for this item.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. On or about July 13, 2018, employees were potentially being exposed to electrical hazards caused by damaged to outer jackets found on electrical cords in which exposed inner conductors. The employer did not ensure that electrical cords were protected from accidental damage as employees were required to use the electrical cords in order to power Spa Pro Massage Stone Heaters. There is no abatement certification or documentation required for this item.
