The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about July 13, 2018, the employer did not ensure established procedures for the application of energy controls were performed in sequence during setup and mold change operations (service and maintenance activities) of wax injection molding machines. Employees did not apply energy controls by; isolating all energy sources [per 1910.147(d)(3) requirements], affixing a lockout device to each energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to starting work [(d)(6)].
