29 CFR 1910.27(b)(1)(i): Before any rope descent system was used, the building owner did not inform the employer, in writing that the building owner had identified, tested, certified, and maintained each anchorage so it was capable of supporting at least 5,000 pounds (268 kg), in any direction, for each employee attached. The information must be based on an annual inspection by a qualified person and certification of each anchorage by a qualified person, as necessary, at least every 10 years. On or about July 7, 2018, employees were exposed to a fall of approximately 50 feet while performing window washing activities on a commercial office building located at 323 Lakeside, Cleveland, Ohio 44113. There was an employee that was suspended from rope descent system and the employer did not get verification from the building owner that the anchor point used could support 5000 pounds.

On or about July 7, 2018, employees were exposed to a fall of approximately 50 feet while performing window washing activities on a commercial office building located at 323 Lakeside, Cleveland, Ohio 44113. There was an employee that was suspended from rope descent system and the employer did not ensure that they were using the proper amount of counter weights.
