29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:  (a)  Most Wanted Roofing LLC, at 1888 Quail Circle, Louisville, CO: On 7/11/18, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  Employees were re-roofing a two story home.  Approximately seven employees were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  This condition exposed the employees to approximate 9.0 foot and 18 foot fall hazards.

Employees were re-roofing a two story home.  Approximately seven employees were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  The employees had not been provided fall protection training.  This condition exposed the employees to approximate 9.0 foot and 18 foot fall hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a)  Most Wanted Roofing LLC, at 1888 Quail Circle, Louisville, CO: On 7/11/18, the employer did not ensure that portable ladders used for access to an upper landing surface had the ladder side rails extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  Employees were re-roofing a two story home.  The employees utilized an aluminum ladder to access the first story roof.  The ladder side rails extended approximately 1.6 feet above the upper landing surface.  This condition exposed the employees to an approximate 9.0 foot fall hazard.
