29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  On or about July 11, 2018, at 1953 Roxey Lane, Winder, Georgia:  Three employees performing roofing work were exposed to struck-by hazards from wood chips, nails, and asphalt shingle debris while using hatchets, and nail guns without wearing eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about July 11, 2018, at 1953 Roxey Lane, Winder, Georgia:  An employee performing roofing activities was exposed to a fall hazard of approximately 13-feet and greater when he worked from the roof ridge without tying off.

29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  On or about July 11, 2018, at 1953 Roxey Lane, Winder, Georgia:  Two employees performing roofing work were exposed to fall hazards of approximately 13-feet and greater when they worked from an approximately 10/12 slope roof while attaching carabiners directly to the webbing of rock climbing harnesses which was connected to a rope grab and rope at the front of their waists.
