29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a. On or about July 13, 2018, at the above referenced inspection site, employees working in a trench approximately 40'x10'x10' were not wearing protective headgear where overhead hazards were present. Employees were setting grade, while an excavator bucket was operated above. Employees were thereby exposed to falling / flying debris and other struck-by hazards. There is no abatement certification of documentation required for this item.

A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a. On or about July 13, 2018, at the above referenced inspection site, employees working in a trench approximately 40'x10'x10' were not provided a safe means of access / egress, in that employees were using an "A" Frame ladder in an unsafe manner. Employees were thereby exposed to falls of 6 - 10 feet from ladder. There is no abatement certification of documentation required for this item.
