29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):      The employer is failing to protect employees from potential fall hazards associated with working from a scaffold platform.  This was most recently documented at 4708 S.48th St. Lincoln, Nebraska on or about July 5, 2018.   The workers were exposed to falls of over 14 feet to the ground below.  Employees were using a tubular framed scaffold which was not fully decked or planked at all working levels.  This exposes the employees to potential fall hazards.

29 CFR 1926.451(c)(2):  Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    The employer is failing to ensure employees are protected from potential hazards associated with working from a scaffold platform.  This was most recently documented at 4708 S.48th St. Lincoln, Nebraska on or about July 5, 2018.  Employees were conducting work off of a tubular framed scaffold which is missing base plates and mud sills from the legs of the scaffold.  The workers were exposed to falls of over 14 feet to the ground below.

29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:       The employer is failing to protect employees from fall hazards associated with working from a scaffold platform.  This was most recently documented at 4708 S.48th St. Lincoln, Nebraska on or about July 5, 2018.   Employees were working on a platform elevated approximately 14 feet which did not have a standard guard rail system.  The employees had installed the cross bracing and was using it as the mid rail, however, there was no top rail installed on the scaffold.

Employees were working on a platform elevated approximately 14 feet which had multiple issues including but not limited to, partially planked working surface, not mudsills and no standard guard rail system.
