29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about July 12th, 2018, at the jobsite located at 10449 NW 82nd St. Bldg. # 8. Highland at Park Central. Doral FL 33178, an employee was exposed to approximately 24 feet fall hazard, while working from an aerial lift basket without the use of fall protection to prevent him from falling out or being ejected from the basket.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  On or about July 12th, 2018, at the jobsite located at 10449 NW 82nd St. Bldg. # 8. Highland at Park Central. Doral FL 33178, the employer exposed an employee working from and aerial lift to lower level fall hazards of up to 24-feet, in that; the employee was not trained on when it was necessary to use fall protection equipment to minimize fall hazards.
