On or about July 12th, 2018, at the jobsite located at 10449 NW 82nd Street, Highland, Building #4, Doral FL 33178, an employee performing wall brick facing was exposed to chemical burns while applying GTS Sanded Thin-Set Mortar (Portland Cement 15-40%, and pH: 10-12.5), without the use of eye protection.    b. On or about July 12th, 2018, at the jobsite located at 10449 NW 82nd Street, Highland Building #4, Doral FL 33178, an employee performing wall brick facing was exposed to chemical burns while applying GTS Sanded Thin-Set Mortar (Portland Cement 15-40%, and pH: 10-12.5), without the use of hand protection.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about July 12th, 2018, at the jobsite located at 10449 NW 82nd Street, Highland Building #4, Doral FL 33178, an employee was exposed to approximately 18 feet fall hazard while working from an aerial lift basket, without the use of fall protection to prevent him from falling out or being ejected from the basket.

Doral FL 33178, the employer did not provide effective training to employees who were exposed to hazardous chemicals such as, but not limited to GTS Sanded Thin-Set Mortar (Portland Cement 15-40%, and pH: 10-12.5), when employees are mixing and applying the chemical product.
