29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about July 13, 2018, the employer failed to ensure that loads lifted by the Crown RC5525-30, serial number 1A376637 were arranged and handled in a safe manner to prevent the load from shifting or falling off the forks. There were two pallets of vacuum cleaners, each pallet had two layers. The top pallet shifted and fell while the forklift was in operation.
