29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: a) Job site, 473 Grant Street, Pittsburgh, PA: On or about July 10, 2018, during scaffold dismantling and other scaffold related work activities, the employer did not ensure that a properly qualified person oversaw scaffold dismantling activities at the job site, resulting in employees being exposed to hazards including, but not limited to, falls of up to approximately 80 feet or greater.

29 CFR 1926.454(c): When the employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer did not retrain each such employee so that the requisite proficiency was regained: a) Job site, 473 Grant Street, Pittsburgh, PA: On or about July 10, 2018, during scaffold dismantling and other scaffold related work activities, the employer did not ensure that retraining had been performed for employees who were not proficient in proper scaffold safety practices. Deficiencies noted during the inspection included, but were not limited to, failing to use appropriate fall protection systems/devices at all times during scaffolding dismantling activities, exposing employees to hazards such as falls of up to approximately 80 feet or greater.
