29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): In the Lathe Department, the employer did not ensure a guard was installed and over the exposed chuck to the Whacheon 18" Chuck Lathe, model WL-720, serial number 8611-04, exposing employees to a struck-by and amputation hazard by the jaws of the chuck. Metal Spinners, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 19106.212(a)(3)(ii), which was contained in OSHA inspection number 1146497, Citation Number 1, Item number 5, and was affirmed as a final order on December 14, 2016, with a final abatement date March 15, 2017, with respect to a workplace located at 1300 Industrial Avenue, Saginaw, Texas 76131.

29 CFR 1904.39(a)(2): The employer did not report within twenty-four (24) hours, the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident. At the establishment, a work-related injury occurred and the employer failed to report to OSHA the inpatient hospitalization prior to the elapse of the 24 hour reporting period of the incident and thereafter.
