The employer was aware of the in-patient hospitalization on June 28, 2018 at around 10:00 A.M. The employer notified OSHA on July 3, 2018 at 12:00 P.M. of the in-patient hospitalization.

The employer was aware that same day that the employee had passed. The employer notified OSHA on July 3, 2018 at 12:00 P.M. of the employee's death.
