29 CFR 1926.20(b)(2): The employer did not provide for a program of frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person: a) Job site, 400 5th Avenue, Pittsburgh, PA: On or about July 10, 2018, during construction activities, the employer did not ensure that ensure that an appropriate system of job site safety and health inspections were conducted by a competent person to identify and abate workplace hazards. Deficiencies noted during the inspection included, but were not limited to, fall protection hazards at window openings, loose guardrail systems around floor holes, missing/incomplete guardrail systems at the edge of interior floors located above 6 feet from the next lower level and a variety of electrical concerns such as romex exposed to unnecessary contact in the workplace, cords laying in the walk/work path, missing romex at junction boxes exposing inner wiring and inappropriate electrical splices.
