29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: 94 Powder Mill Road, Southwick, MA: On or about July 16, 2018 employees were observed working in a trench box that did not provide adequate protection from falling rock and debris because it did not extend the required 18 inches above the vertical face of the excavation when the box was installed below the surface of the excavation.
